About transferring from a community college?

I want to be a pharmacist, right now i am senior in HS, i want to go to Onondaga community college for the first 2 years, is that possible to transfer to Albany pharmacy college after 2 years of my study's in OCC. And please help me with what type of credits i should get in OCC to transfer to ACPHS. Please help me!!!!!!!!!!
